Reduced Fare Card Application (Fixed Route)

  1. Connect Douglas offers reduced fares for the fixed route service to the disabled, seniors over age 60, and current students of public or private schools, colleges or universities. Riders may present a valid ID directly to the bus driver or complete the Reduce Fare Card application and receive a Reduced Fare ID Card.

    Submit this online application with the required  verification documents attached or visit the Transportation Center (located at 8800 Dorris Road, Douglasville GA) to present the required documents and have your photo taken for your ID card. You may also email documents or photo to: .

    If all components of the application (including attached documents and photo) are submitted online, Connect Douglas staff can print and mail your Reduced Fare badge to your home address.

    Please allow up to 14 days for your application to be processed. After 14 days, please call Connect Douglas Customer Service at 770-949-7665 to check on the status of your application.

    Select the type of proof of disability you will upload. You must also provide one of the following:

  6. Attach a Medical Verification/Disability Form, Disability Card from another transit agency, SSI/SSD letter,  or Medicare Card.

  7. Please upload a clear picture or photocopy of your Student ID card. You may also upload enrollment document, recent report card, or another document that shows proof of your current enrollment at any school or educational institution.

  8. Upload a photo of yourself from the shoulders up against a solid background. This photo can be used on your Reduced Fare ID Card.

  9. Fixed Route Bus with deviation/ADA Paratransit/Vanpool/SRTA Xpress persons with hearing or speech disabilities who need to contact Douglas County may place their call through the Georgia Relay Center at 800-255-0056 (Text Telephone) or 800-255-0135 (Voice Telephone).
